Job IntroductionWe have an exciting opportunity for you to join our forward thinking, rights based Adults with Disabilities service. We have a team of skilled, dedicated, friendly and supportive staff who are committed to upholding people’s human rights through their social work practice. However, as our service evolves, we are looking for a new Advanced Practitioner Social Worker colleague to join us and continue to develop and provide a high quality service for the people we support. The team will be based in a locality of Bradford, providing long term support to around 600 adults with physical disabilities, learning disabilities and neurodiverse conditions with eligible needs under the Care Act 2014. The team will consist of a Team Manager, an Advanced Practitioner and up to 8 social workers and/or care coordinators.You will support the Team Manager, as their deputy. You will have a strong commitment to human rights at the heart of social work practice and will know and understand the importance of the UN Convention on the Rights of the Child and the UN Convention on the Rights of Persons with Disabilities. You will not want to make any decision without the person you are there to support being in control and being enabled to fully participate.The main responsibilities of this role:
