• Developing tailored care plans and delivering one-to-one support.
• Collaborating with the prescribing team to deliver harm reduction and specialist interventions.
• Supporting individuals to achieve stability, reduce harm, and regain independence in their communities.

What You’ll Bring

• Relevant experience in the substance misuse field.
• A degree in Psychology, Sociology, Social Work, or a Diploma in Health and Social Care (or equivalent).
• Extensive experience in the field may be considered in place of formal qualifications, with access to accredited training.
