We’re seeking compassionate and dedicated nurses to join our dynamic team, delivering care right in the heart of the community–directly in patients’ homes and residential facilities.As a valued team member, you will assess and develop management plans for complex and acutely unwell patients. Your expertise will guide unscheduled nursing care, ensuring our priority patients receive timely and effective support.What We Need:

• Registered Nurse with current NMC registration
• Nursing degree or equivalent
• Substantial years of post-qualification experience

Desirable

• V300 prescribing non-medical prescriber
• Non-Medical Prescribing qualification (V150)
• Community Nursing Specialist Practitioner qualification
• Community experience

Knowledge and SkillsEssential

• Demonstrate evidence based clinical knowldge
• Effective communication skills
• IT skills
• Proactive problem solving
• Ability to travel to different geographical locations across the Trust footprint in line with service needs

Desirable

• Leadership skills
• Full UK driving license with willingness to use own car for work purposes
