Your Responsibilities
What you’ll do
The post holder will need a keen interest in the timely and efficient discharge process of patients with complex needs, undertaking the assessment and discharge planning of patients within the defined clinical area, working under direction of the Complex Discharge Lead, using their clinical knowledge and experience to support other members of the team, completing their own assessments and interventions where indicated. The post holder will also be responsible for the management of identified B2-4 staff, to carry out supervision including appraisals.
What we’re looking for
You will be expected to be a confident communicator with excellent organisational skills. You will be required to liaise with family members, patients and members of the Multidisciplinary Team on a regular basis and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and pressurised environment, be able to provide highly specialised clinical discharge planning advice both within the Trust and externally, to other team members, and to health and social care professionals.
