• Location: Woking
• Duration: 3 – 6 months ongoing
• Schedule: Mon – Fri, 9 – 5
• Start Date: August 2025
• Rate of Pay: £25 – £27 per hour

What You’ll Be Doing:Join our team as a CPN and play a key role in improving the lives of working age adults. In this role, you will:

• perform comprehensive patient assessments
• create and implement personalised care plans
• deliver evidence-based treatments to optimise patient outcomes
• collaboration with multidisciplinary teams to provide holistic care
• maintain accurate patient records
• visits patients out in the community

Requirements & Qualifications:To be successful in this role, you will need:

• a current and valid NMC registration and pin
• experience working with working age adults in the community
• strong communication and interpersonal skills
• the ability to work independently and within a multidisciplinary team
• a commitment to delivering high-quality patient care
• access to a car for community/home visits

Working Hours & Contract Duration:This role offers a flexible working position requires availability from Monday – Friday with flexibility where possible. The contract is for 3 to 6 months with a strong possibility of extension based on performance and service needs.This opportunity won’t last long!
