A new opportunity has arisen for a Mental Health Care Coordinator to join our team as part of the Picton Primary Care Network (PCN) NHS. This is a hugely exciting opportunity to work innovatively across our network to support the growth of our Mental Health support services.The successful candidate will report to our Network Lead and manage multiple mental health workstreams, including administrative and project management duties. Part of this role will be helping to lead in a perinatal mental health project which we have received funding for.The two most important attributes that we are looking for in a candidate is excellent communication skills and a true passion for helping improve peoples wellbeing. Experience of working or volunteering in mental health would be preferable.Main duties of the jobThe Mental Health Care Coordinator will provide management and support to our Network team and the 6 GP practices. They will be responsible for bringing together the various services that we offer to patients and will help lead improvement and innovation. The main way in doing this will be through facilitating the weekly mental health mdt, which sees our mental health team come together to discuss complex cases and go through referrals. The ideal candidate will have experience of leading meetings, including the set up and follow-up actions from each meeting.They will be required to communicate effectively and sensitively to patients, clinicians and practice admin staff across the Network GP practices.The successful candidate will also work with our Network Lead to engage our patient population and deliver wellbeing events, drop-ins and workshops across the local community. They will build on existing relationships, and form new ones, with mental health and wellbeing services across Liverpool.Additionally, the Mental Health Care Coordinator will be responsible for service evaluation by analysing data and reporting to the Network management.About usPicton PCN consists of 6 GP practices that cover 50,000 patients.We have a large Network team that consists of Pharmacists, Physios, Paramedics, Social prescribers, Care Coordinators, and a Mental Health team. We work together to provide quality care by delivering clinical appointments in our GP practices and health events in the community to our patients.Pictons mental health team cover a range of mental health conditions including low mood, anxiety and depression, up to the more complex cases such as personality disorders.
