If so this is the post for you.An exciting opportunity has arisen within the East Sutherland Health and Social Care Team. We are looking for a motivated & enthusiastic individual who will be part of a multidisciplinary team working with patients in their own homes to provide holistic care. Your primary responsibility will be to assist with enabling people to maintain their independence at home and supporting people after they have left hospital.
