The post will assist the Health and Local Crisis Response Service Coordinator by undertaking duties required to meet the service users’ needs in order to re-enable service users to remain in their discharge location and prevent hospital re-admission. The post-holder will be required to have the ability to assess service users’ needs, complete and follow a support plan for the individual. The post-holder will be providing practical and emotional support in the service user’s discharge location and signposting to other services where appropriate to enable the individual to live independently in their accommodation.
The Support Worker will act as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area.
