The social prescriber link worker will provide a holistic approach to healthcare by supporting individuals to improve their overall health and well-being through access to non-medical, community-based support. Social prescribers focus on the wider determinants of health such as loneliness, housing, financial challenges, and lifestyle factors working alongside individuals to address the social, emotional, and practical issues that impact their health.Main duties of the job

They will receive monthly clinical supervision and have access to support for safeguarding, domestic violence, and mental health concerns. GP referrals will be recorded using SNOMED CT codes in clinical systems.
