DaVita is looking for a Licensed Social Worker (LMSW) to support our patients and their families at our Waters Place Dialysis Clinic in Bronx, NY!

• Covering Waters Place Dialysis Clinic (1733 EASTCHESTER RD, BRONX, NY 10461)
• 8 AM – 5 PM / Monday – Friday schedule
• Full time schedule – 32-40 hours

• Life on dialysis can be a difficult transition for many patients-and you’ll be there to support and advocate for them.
• Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Be a part of a Team that appreciates, supports and relies on each other in a positive environment.

Requirements:

• Licensed or registered in the state of practice as required by state regulations — LMSW or LCSW
• Meets all state required regulations to practice in the dialysis setting.
• Master’s degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

• Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
