Job Summary:
The Rehabilitation Admissions Liaison is the clinician responsible for promoting positive public relations between Virtua Lourdes Regional Rehabilitation Center and other health care agencies as well as acting as a liaison between health care team members, prospective Rehabilitation candidates, health care facilities, and insurance companies.
Position Responsibilities
Evaluates potential patients for admission to acute rehabilitation- performs a thorough chart review to identify appropriateness for admission. Speaks to patient, families, discharge planners, physicians and therapists as necessary to obtain a detailed evaluation of the patient. Identifies any potential clinical issues they may be pertinent to the rehab stay. Anticipates the patient’s expected length of stay and any potential barriers for the patient to return to the community. Ensure completeness of the preadmission assessment as required by Medicare regulations
Facilitates admission process to the acute rehabilitation unit. Communicates with rehab physicians to ensure they have an accurate assessment of the patient, to make an admission decision. Coordinates with insurance verification to ensure coverage. Assists discharge planners in obtaining insurance authorizations. Coordinates with the nursing unit to ensure clinical capabilities and any necessary equipment.
Ensures that the preadmission screen is signed by the admitting physiatrist as per CMS regulations
Identifies opportunities to increase referrals/admissions to acute rehabilitation. Provides educational in-services to discharge planners and attends various community and professional events to increase awareness of the acute rehabilitation services.
Position Qualifications Required:
Required Experience: 2-4 years related experience preferred
Required Education: Bachelor’s degree required; master’s degree preferred
Training / Certification / Licensure: Current driver’s license; License within a professional clinical discipline (RN, PT, OT, SLP, LSW)
