Summary of Job Duties:
The Outreach Coordinator (OC) plays a crucial role in ensuring that patients and community members receive the necessary education and support to overcome barriers related to cancer prevention, education, and care. Working as part of a dynamic team, the OC acts as an intermediary, advocate, and supporter, working closely with patients and their care teams.
In this position, the Outreach Coordinator will focus on building relationships with patients and community members, providing them with the necessary knowledge and resources to improve their health outcomes. Providing community outreach and cancer patient navigation in Franklin, Johnson, Logan, Scott, Sebastian, and Yell counties. They will be instrumental in connecting individuals to cancer prevention programs, educational resources, and healthcare services. They will also provide support, guidance, and case management services to patients, helping them navigate their care journey and address any social support services they may require.
Collaborating closely with a registered nurse case manager, the OC will actively participate in the patient’s care team. Their role will involve creating personalized goal plans, coordinating interventions, and providing individual and group education and support. By working within the community, the OC will have the opportunity to engage with individuals directly, providing outreach and support in various locations outside of the office environment.

Qualifications:
Minimum Qualifications
- Associate’s degree plus four (4) years of experience in outreach education or administrative experience.
- A valid driver’s license with current liability insurance is required.
- Must have a working cell phone.
- Must reside in any of the following counties: Franklin, Johnson, Logan, Scott, Sebastian, or Yell.
Knowledge, Skills, & Abilities:
- Strong communication skills: The OC must be adept at effectively communicating with patients, community members, and healthcare professionals. They must be able to convey complex information in a clear and concise manner.
- Empathy and Compassion: The ability to empathize with patients and community members is essential. The OC must be able to listen actively and provide emotional support while addressing their needs and concerns.
- Resourcefulness: The OC must possess the ability to identify and navigate various resources within the community, including healthcare, social services, and educational organizations.
- Organizational skills: The OC must be detail-oriented and have excellent organizational skills to manage multiple tasks and responsibilities. They must be able to prioritize and manage their time effectively.
- Strong advocacy skills: The OC must possess the ability to effectively advocate for patients’ rights and needs, ensuring they receive appropriate resources and support.
- Remote work adaptability: This position is primarily remote, requiring the OC to work effectively and independently in a virtual environment.
- Intermediate Proficiency in Microsoft Office suite.

- Bachelor’s degree plus two (2) years of experience in outreach education or administrative experience.

Additional Information:
Key Responsibilities:
- Provide community outreach and cancer patient navigation in Franklin, Johnson, Logan, Scott, Sebastian, and Yell counties.
- Maintain a full caseload via one-on-one interactions and through group education and support.
- Work with families/individual patients to set goals.
- Facilitate workshops with an emphasis on cultural competency and adherence to grant non-discrimination policy.
- Maintain a working knowledge of community resources.
- Display an ability to multitask and prioritize competing deadlines and projects in a fast-paced work environment.
- Developing content for and coordinating group educational and support activities.
- Assisting in identifying gaps in services and health needs, and devising remedies in health-related areas, particularly cancer support.
- Follow-up with patients who miss appointments or education sessions.
- Document service delivery and conduct periodic service plan meetings with participants and service providers.
- Administer & collect data surveys to participants.
- Maintain accurate workshop data, including attendance rosters, evaluations, and other reporting documents.
- Use identified tools to accurately track and report daily activities.
- Provide linkages to community resources such as cancer health services, housing, workforce development programs, transportation, food, and mental health services.
- Collaborate with faith-based organizations, community organizations, and other community leaders and agencies to co-facilitate workshops and conduct outreach.
- Host and attend community events and festivals.
- Coordinates community outreach efforts through presentations and multi-media.
- Assisting and/or presenting programs on health information, particularly cancer support, and services to community groups.
- Work with the UAMS communications team to create/distribute promotional materials for partners, community agencies, and targeted audiences.
- Training, team meetings, administrative responsibilities, and other duties as assigned.
