NHS Devon are seeking to recruit a Continuing Healthcare (CHC) Assessor to work in either the Plymouth/western area or the Exeter/Eastern area of Devon. This role requires home working with regular attendance at the office base in Exeter.The post holder will be part of the Nursing and Quality Directorate for NHS Devon Integrated Care Board (ICB). Their aim will be to improve the quality of care and value for money of services commissioned for individuals who are the responsibility of NHS Devon ICB.The post holder must be a registered nursing professional or Allied Health Professional/Social Worker and have an ability to travel independently around Devon and the wider locality. (Travel expenses and travelling officer status available). We are also interested in individuals who have learning disability experience.Main duties of the jobTo undertake CHC assessments of individuals who have been identified as potentially eligible for NHS funding contribution to their care. These assessments are to be undertaken in a timely manner and in variety of settings, including the individual’s own home, care home setting, other setting or virtually, as required.Responsibility for facilitating, assessment and reviewing a person’s eligibility for NHS CHC Funding or Funded Nursing Care (FNC) contribution ensuring that the core values and principles of the National Framework is applied, and all local policies and procedures are adhered to.Completion of a suitable personalised care plan drawn up for and with the individual, and where appropriate through a Personal Health Budget, (PHB) considering the ICB’s responsibility to arrange safe, good quality care that meet the needs of the person and make the best use of available resources.To actively engage with a wide range of stakeholders including staff, service users, their carer’s, and other statutory and voluntary sector agencies to ensure people are fully briefed, know and understand commissioning and care management processes in relation to CHC, FNC and health and social care joint funded packages of care.To provide information and explanation to individuals’ relatives, carers, representatives, and advocates, regarding the assessment and review processes. - Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area.
- Experience of case management.
- Experience in care planning and assessment of need.
- Evidence of continued professional development.
- Member of a relevant professional body. (Registered with NMC or equivalent to specialist health profession) or Allied Health Professional (AHP) essential)
- Experience of working as an autonomous practitioner, managing own workload and identifying areas for development.
Desirable
- Previous community experience
- Previous experience in similar role, including Registered Nurses, Allied Health Professionals and/or Social Workers
- A good understanding of the health and social care environment and roles and responsibilities within it
