As a Case Manager, you’ll play a key part in providing long-term case management to adults facing substance misuse challenges. Your responsibilities will include:
- Developing tailored care plans and delivering one-to-one support.
- Collaborating with the prescribing team to deliver harm reduction and specialist interventions.
- Supporting individuals to achieve stability, reduce harm, and regain independence in their communities.
What You’ll Bring
- Relevant experience in the substance misuse field.
- A degree in Psychology, Sociology, Social Work, or a Diploma in Health and Social Care (or equivalent).
- Extensive experience in the field may be considered in place of formal qualifications, with access to accredited training.
