Work in collaboration with Bevan colleagues to deliver a coordinated and high-quality service that proactively encourages service users to access and engage with community support. To work as part of the Health Inclusion Outreach Team through attendance at Shelters, Soup Kitchens, Volunteer/Charity Sector locations, HMO accommodation and more.Please note there is one full time and one part time vacancy.Main duties of the job

  • Manage safeguarding concerns appropriate to your role and level of training.
  • To plan, develop and facilitate group and 1-2-1 sessions.
  • Offer comprehensive support to empower people to take control of their health and wellbeing. You will provide social, emotional, and practical support tailored to the service users needs and facilitate access to community services, pathways to recovery, housing solutions, social reintegration, and mental health support.The role is not intended to support individuals long term, but to help them to understand how they can support themselves better by exploring barriers to improved mental, social, and physical health.
  • Contribute to the development of the service and participate in support, supervision and training as required.
  • Contribute to the education of staff within the network and maintain details and grow working knowledge of sources of support in the community.

  • Develop trusting relationships with service users, giving them time to focus on their assets and what matters to them and providing them with personalised help to take control of their health and wellbeing, live independently and better understand the impacts of their lifestyle choices.
  • Undertake holistic client needs assessments using a wellbeing template. Support, training and supervision will be given.
  • Support a caseload of clients for whom wellbeing support and mentorship might offer improved outcomes. With the team, proactively identify people who would benefit from this type of help and manage and regularly review your caseload to accommodate urgent referrals for support as required, referringout to community support services as soon as possible.
  • With clinical support, develop a strong awareness and understanding of when it is appropriate or necessary to escalate care back to the GP or other care professionals when the persons needs are beyond the scope of the health and wellbeing support worker e.g. when there is a mental health need requiring a qualified practitioner.
  • Understand when appropriate to undertake clinical skills and assessments on clients and provide clinical skills outside of a clinical setting. I.E Phlebotomy, baseline observation, spirometry.
  • Provide clinician led care, under direct and indirect supervision of a clinician. I.e wound care, vaccinations etc.
  • To be pro-active in maintaining personal and professional learning and development and identify own training needs as they arise.
  • To attend team meetings, away days and other similar events.
  • To participate fully in regular one-to-one supervision sessions and in an annual performance appraisal.
  • Work as part of the team to seek feedback, continually improve the service, and contribute to business planning.
  • Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
  • Ensure compliance with all relevant policies and procedures in relation to health and safety, equal opportunities, safeguarding, lone working, customer service, data protection and confidentiality.

  • Experience of assessing need and working within time-limited interventions to promote and support change.
  • Experience of planning, facilitating, and evaluating workshops / groups, focused on self-help.

  • Excellent communication skills, both verbally and in writing.
