• Powys, Wales
• Adult Mental Health
• Working within an established, integrated, multidisciplinary team, focusing on individuals with severe and enduring mental health problems
• The post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clinets with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010
• A high standard of nursing care will be required to facilitate the Care Co-ordination role
• Duration: ASAP until 1st April 2021
• Up to £25 per hour depending on experience

Requirements

• Experience working within a community setting and with severely mental ill clients
• Sound understanding of community and multidisciplinary work
• Diploma level education in one of the following: RMN or OT or SW
• DBS or Police Check
• Registered with relevant goverment body
